The billing policy for Illinois Plastic Surgery requires full payment of fees as quoted thirty (30) days prior to your surgery.
We accept cash, check, and major credit cards, or we can provide you with information about various other financing solutions. Our professional administrative staff will be happy to discuss the options with you, including a variety of down payment and monthly payment choices. Your health is of utmost importance to us. Therefore, to ensure that we have all the needed information to properly care for you, we have several patient information forms for you to complete. Be sure to select the forms that are used by your specific physician. Please remember that accuracy and honesty are extremely important when filling out any official paperwork, especially that related to your health.

2. Print forms, complete, sign and bring them with you to your appointment.
